Regarding Absence from Classes or Regular Exams due to Adverse Reactions after Vaccination

To students,

If it is unavoidably necessary for a student to be absent from a classe or a regular exam owing to the symptoms that are observed as adverse reactions due to vaccination (in Japan) and if the student wishes to consult the Academic Affairs Division, please take the procedure below and contact PEAK/GPEAK Section via e-mail as soon as possible.

<Classes>

・Report the symptoms through "COVID-19 Infection Report Form", and consult a medical institution promptly.

・Explain the circumstances and request consideration directly to the instructor in charge of the class.

<Regular Exams>

・Report the symptoms through "COVID-19 Infection Report Form", and consult a medical institution promptly.

・Submit a document that shows the date of vaccination and a medical certificate to PEAK/GPEAK Section immediately.

(Academic Affairs Division will inform you how to submit the documents.)

・Alternative measures may be considered based on the documents you have submitted.

It is not allowed to take alternative measures if you take the procedures after the start of the exam.

We recommend that you select the day of vaccination not to affect the classes and regular exams.
