Registration Forms for Sub-programs and Others AY2018
The students must submit a notification form below if they would like to register for a Sub-major program, Sub-program and/or Cross-disciplinary Program.
Before submitting the notification sheet, the approval (signature or personal seal (Hanko)) from the Program Director of JEA or ES to which you belong must be obtained.
