Registration Forms for Sub-programs and Others AY2023
Students must submit a notification form below if they would like to register for a Sub-major program, Sub-program and/or Cross-disciplinary Program.
Before submitting the notification sheet, the approval (signature or personal seal (Hanko)) from the Program Director of JEA or ES to which you belong must be obtained. Plan well in advance to complete the procedures within the designated period.
